Who we are

TBF Diagnostic provides drug and alcohol screening, DNA collection, blood draws, Live Scan fingerprinting and Health Care Worker Registry fingerprinting at 9150 Crawford Ave, L-1 (Basement), Skokie, IL, 60076, plus scheduled mobile on-site services. What we collect

• Contact details you give us when booking or creating an account: name, phone number and email address.
• Booking details: the service, appointment date and time, and a booking reference.
• Corporate account details: company information and the employees a corporate administrator registers.
• Employee portal data: assignment location, clock-in and clock-out times with GPS verification, and documents an employee chooses to upload.
• Payment records: confirmation of completed payments, handled by PayPal or QuickBooks.

We do not ask for your date of birth when booking. Test results are never collected, stored or displayed through this website.

How we use it

We use this information to schedule and confirm appointments, prevent double bookings, take payments, notify our office of bookings, clock-ins and uploaded documents, and create calendar entries for confirmed bookings. Calendar entries contain only the customer name, contact, service, time and booking reference.

Payments

Online payments are processed by PayPal and QuickBooks Payments. Card details entered at checkout go directly to the payment processor and are never stored on our website. We record only that a payment was completed and for which booking.

Sharing

We share the minimum information needed with payment processors (PayPal, QuickBooks), our email and calendar provider, and licensing or registry partners when a service requires it, such as Health Care Worker Registry fingerprinting submissions. We do not sell personal information.

Retention and your choices

Booking and payment records are kept as long as needed for business and legal purposes. Uploaded employee documents are stored privately and shared with our office only through links that expire. You may request access to, correction of or deletion of your personal information by emailing info@tbfdiagnostic.com.
