Using this website

By booking a service, creating an account or using this website, you agree to these terms. The website lets you view our service catalog, book appointments, pay for eligible services online and, for authorized employees and corporate administrators, use the employee and corporate portals.

Bookings and appointments

Choosing an open time books your appointment immediately. Appointment times are held on a first-come basis and confirmed by email. Please arrive on time for walk-in and booked visits at 9150 Crawford Ave, L-1 (Basement), Skokie, IL, 60076. Mobile on-site services are scheduled in advance at the workplace location agreed with us.

Payments

• Online payments are charged in US dollars at the price shown at checkout: the fixed price for fixed-price services, or the starting fee for services priced "from".
• Services priced "from" may require an additional balance for extra panels or agency fees, payable at the site.
• "Payment accepted onsite" services are paid at the time of your visit.
• Payments are processed by PayPal or QuickBooks Payments. We never see or store your full card details.
• Keep your payment receipt — our office confirms payment against your booking reference.

Accounts and portals

You are responsible for keeping your account password confidential and for activity under your account. Corporate administrators are responsible for the employees they register. Employees must provide accurate assignment locations when clocking in, and clock entries record verified GPS distance to the assigned or office location. Do not upload documents that are not yours or that you are not authorized to submit.

Testing services

Collection and screening follow the applicable chain-of-custody and agency requirements. Results are released through the appropriate laboratory, agency or registry, not through this website. Our services are not medical care and do not replace advice from a licensed health care provider.

Changes and cancellations

Need to change a booking? Call (224) 999-7624 during business hours and we will reschedule you. We may update these terms from time to time; the current version is always posted on this page.
